Health professionals in our community are a critical part of the My health for life program.

We value strong, collaborative relationships that support the best health outcomes for Queenslanders.

Our program is a practical extension of the advice you give your patients/clients.

We work with program participants to help them:

  • understanding their personal risk of chronic conditions including type 2 diabetes, heart disease and stroke
  • how lifestyle choices impact their overall health
  • how to make behaviour change in manageable steps
  • goal setting and action planning for a healthier lifestyle
  • maintaining good health.


Wanting to refer your patients?

Referring your patients/clients into the program is a simple process.

Please note: Do not open the templates until they have been imported into your clinical software as this will corrupt the coding. For assistance importing these templates please contact your PHN Practice Support Officer or visit your PHN's website.

We ensure that medical practitioners who have patients participating in the program are kept informed of their patient’s progress, completing the care cycle.

If your patient/client would like to know more about the program, direct them to our website where you can take them through the health check, alternatively they can call our team on 13 74 75 who will take them through the health check and provide more information on what the program can offer.

Our team of health professionals

My health for life group facilitators and telephone health coaches come from a variety of healthcare backgrounds and include exercise physiologists, dietitians, nurses, psychologists, GPs and more. We have a network of over 120 experienced health practitioners across Queensland who have completed training to ensure they are equipped to guide and support participants through the program.
